The Region of Eindhoven and De Kempen Care Circles

The care sector is under pressure due to an increasing number of elderly and chronically ill people and the looming shortage in health care staff.

In the Eindhoven and Kempen region, health care organizations and educational institutes therefore joined forces to organize care and prepare for the future. The first project is the organization of night care. This project has three pillars: joint teams for night care, the use of domotica (home automation) and the support from a central exchange.

Night team
Healthcare organizations will jointly organize their night shifts. For each village (or neighbourhood) or for some towns (districts) together, an overnight team is installed. The staff of the local team are on call all night. If a client is in need of support, a team member can quickly respond. The team works for people who are connected to a personal alarm system and provides care to clients of the member organizations. The staff is trained in order to be able to work for an extensive target group.

Monitoring
Through the use of technology it is possible to organize monitoring in the client’s own home and thus enhance the security at home. Detectors are installed in the house (e.g. motion detectors, fire alarms or door contacts). These will alert the care call centre when something is wrong. Thus it is possible to monitor whether someone has fallen, if there is a fire or whether someone is wandering about. The number of homes with care domotica will be extended.

Care call centre
The technical applications at home are connected to a care call centre. The operators take care of the first reception and pass on the signal. Depending on the support needed and the agreements made, caregivers, medical professionals or emergency services are contacted. Healthcare providers agree to make use of the same regional care call centre.

Alignment
Every healthcare provider has its own rules and regulations, procedures and dossiers. The joint organisation of care calls for alignment and adaptation. This project provides the opportunity to test this. It is expected that in due course the alignment will improve. At the end of the project in December 2011 at least 4 care circles will be operational; for some more care circles preparations will have been made. If the experiences prove to be positive, further expansion will surely follow.
