Healthy at home in Breda

To advance social contact and welfare, that is the core of the Healthy at Home project in Breda. A group of 250 elderly and chronically ill people with a need for support are connected in a network, together with one thousand caregivers; a so-called social community.

They can also make use of services related to housing, entertainment and health & care. All this is available through an easy and user-friendly application. Upon installation the users receive a detailed instruction. Moreover, the helpdesk is always at their disposal.

Playful training
The elderly work on a touch screen, while caregivers get access via their own PC. Experience shows that new users do nothing else than playing games during the first three weeks. Initially against the computer itself, but then also against opponents in the online network. Thus, they gradually familiarize themselves with the system. Which makes them skilled users when the time comes that they are in need of other services than entertainment.

Good morning’ service
People also use the system to contact their family and friends and other people in their neighbourhood. They also communicate with care providers. For example, they make arrangements with others to go for a walk. Caregivers can use a central agenda to coordinate their care services. And every morning there is the 'good morning service’, a moment of video contact between client and caregiver at the beginning of each day.

All-in-one package
The complete package of the Healthy at Home services consists of remote care by means of ‘tele-support’ for the chronically ill with, for example, diabetes, prevention programmes such as lifestyle counseling and a video consultation hour. The system also provides access to regional and local information such as the neighbourhood events calendar. A link to security services is also possible. Initiator Careyn Thuiszorg (home and health care provider) hopes that in the near future the system can also be used to communicate with e.g. municipal services and departments.

Linking to other providers
It is intended that from 2012 Healthy at Home will be available to all residents of Breda in need of this service, by means of a subscription. Careyn will endeavor to connect their system with those of other health care providers. Users must be able to contact each other through video communication services regardless of the provider.

In this project Careyn Thuiszorg Breda cooperates with WIJ Foundation, Foundation Steunpunt Informele Zorg Breda (centre for informal care), PAL4 B.V., Foundation Goed Geregeld (centre for home care), Elipz Foundation (centre for innovative primary care), University of Tilburg and Avans University of Applied Sciences.
