Early memory support
If caught early, you can ensure that people who have been diagnosed with early memory loss continue using regular care services. The alternative, intensive care provision, is not only expensive but scarce. It all starts with signals indicating unusual behaviour: the first signs that elderly people are experiencing memory loss. Stepping in on time and appropriate support enable people to live in their own environment for longer.

• Essence of the project: People who are in an early stage of dementia can be helped more easily, enabling them to live independently for longer. By recognising and consolidating small changes in their behaviour, it is possible to help them earlier and more efficiently. It is crucial that we engage the social circle surrounding the elderly as they will recognise the first signals. Using technology, signals sent by several parties from the social circle can be combined and mapped making it possible to form a profile of the individual.
• The different project partners are conducting a user survey amongst elderly people, carers and parties who are often in contact with the elderly. In addition, they are investigating the rules and regulations that they should adhere to, the way in which the process should be set up and the available techniques and technologies that they are able to use. Strength of this project: focus on the needs of all parties involved. The partners combine their personal expertise and optimally make use of it to set up an implementation plan.
• With this project, the different partners would like to ensure that elderly people who are experiencing initial memory loss do not fall between the cracks. Due to an established coordination between organisations, an effective healthcare chain will be established around the individual client.
This is a project by: Land van Horne, Wonen Weert, Budels Medisch Centrum, the municipality of Cranendonck, Institute for Paramedical Studies at Fontys University of Applied Sciences and Simpact BV. It is aimed at the residents of the Cranendonck region.
